Financial Advisor Forms

Follow these simple steps for adding our FEBA Registration Form and FEBA Evaluation Form to your website or sending directly to clients:

CREATE YOUR OWN FORMS

  1. Go to Google Forms
  2. Click “Blank” under “Start a new form”
  3. Title the form based on registration or evaluation
  4. Add the questions listed under each form below
  5. Contact information will require “short answer” format
  6. Evaluation questions will require “linear scale” format of 1 to 5
  7. Anything with a red asterisk is required– click the required slider on corresponding questions
  8. Once complete, click “Send” to access URL and embed options for sharing
